Discharge Policy
At Aadhar Health Institute, we believe that smooth discharge planning is essential for your safe recovery at home. Our aim is to guide you and your family at every step.
Your treating doctor, nurses, resident medical officer, front office, billing team — all work together to make your discharge process hassle-free.
- Advance Notice:
Your doctor starts planning your discharge once your condition improves. Usually, this is done at least 12 hours in advance. - Medical Fitness:
You will be discharged only when your doctor decides you are stable enough to go home safely. - Multi-disciplinary Team:
If more than one specialist was involved in your care, all will review your discharge summary.
Before you leave, you will get:
- A Discharge Summary containing your diagnosis, test reports, treatment details, and any procedures done.
- A list of medicines, doses, and instructions.
- Advice about diet, rest, wound care, or any special precautions.
- Follow-up dates and doctor appointments, if needed.
- Instructions on how to reach the hospital if any urgent care is required.
A copy of this summary will be safely kept in your hospital file.
If you need extra help after discharge — like home nursing, rehabilitation, or help for elderly family members — our team will guide you and your family.
- The ward nurse or front office will inform you about any remaining payments.
- Please clear all bills before collecting your discharge card.
- For cash payments, discharge usually takes about 3 hours.
- For insurance, TPA, Ayushman Bharat, or government schemes, it may take 5 hours as approvals are processed.
If you choose to leave the hospital before your doctor advises:
- The doctor will explain the possible risks.
- You or your family must sign a consent form.
- You will still get a summary and advice for further care.
If a patient passes away, the family is supported with clear documentation, including a detailed death summary and required certificates. All legal formalities are managed as per guidelines.
- Please follow the instructions given in your discharge summary.
- Keep your follow-up appointments on time.
- If you face any unexpected symptoms, contact your doctor or visit our emergency services immediately.
