Complex Gallbladder and CBD Case Successfully Treated with Advanced Lap. Surgery
When a 70-year-old diabetic male arrived at Aadhar Health Institute, Hisar, he was suffering from severe right upper abdominal pain that had been worsening over several days. On evaluation, imaging studies revealed a gallbladder perforation with multiple stones in the common bile duct (CBD) — a rare and serious condition requiring urgent surgical management.
The patient had previously undergone ERCP with CBD stenting for stone removal a few months earlier. However, follow-up MRCP showed that new stones had formed, along with thickening of the gallbladder wall and a focal perforation area, indicating a high risk of infection and bile leakage.
After a detailed multidisciplinary discussion, the surgical team planned a complex laparoscopic procedure —
Lap Cholecystectomy with CBD Exploration and Stent and Stone Removal with CBD Repair over T-Tube.
During surgery, multiple stones were successfully removed, the CBD was cleared, and a T-tube was placed to ensure smooth bile drainage and healing. The procedure was performed entirely laparoscopically with minimal blood loss.

The patient recovered well postoperatively, with no complications. He was discharged in stable condition and showed excellent recovery during follow-up visits, with good T-tube drainage and complete symptom relief.
This case demonstrates how advanced laparoscopic expertise and timely intervention can prevent life-threatening complications in elderly and diabetic patients with complex biliary diseases.
