Laparoscopic mesh repair of a large type IV hiatal (Diaphragmatic) Hernia
Introduction
Type IV hiatal hernias are complex diaphragmatic defects characterized by the herniation of the stomach along with other intra-abdominal organs into the thoracic cavity. This report details the successful minimally invasive management of a giant Type IV hiatal hernia in an elderly patient.
Patient Presentation
A 73-year-old male presented to the surgical outpatient department with a history of recurrent vomiting, severe epigastric pain, and episodes of hemoptysis. His symptoms had been progressively worsening, significantly impacting his nutritional intake and overall quality of life.
Diagnostic Evaluation
The patient underwent a comprehensive diagnostic workup. Upper gastrointestinal endoscopy and high-resolution computed tomography (CT) of the chest and abdomen confirmed a large Type IV hiatal hernia. The hernial sac was extraordinarily large, containing the stomach, segments of the colon, and a significant portion of the greater omentum, all migrating upward through the hiatus into the posterior mediastinum.
Surgical Management
The patient was scheduled for a minimally invasive laparoscopic repair. The key operative steps included:
- Reduction of Contents: Careful and complete laparoscopic reduction of the herniated stomach, colon, and omentum back into the abdominal cavity.
- Cruroplasty and Mesh Reinforcement: Primary closure of the widened diaphragmatic crura. Given the massive size of the defect and the patient’s age, a mesh reinforcement was securely placed over the cruroplasty to minimize tension and reduce the likelihood of recurrence.
Postoperative Course
The minimally invasive approach facilitated a rapid and remarkably smooth postoperative recovery. The patient was mobilized early on the surgical ward, gradually tolerated oral intake without any obstructive symptoms, and experienced a complete resolution of his preoperative vomiting, pain, and hemoptysis.
Conclusion
Laparoscopic reduction and mesh-reinforced cruroplasty offer a highly effective, safe, and definitive treatment for giant Type IV diaphragmatic hernias, even in geriatric patients with
complex organ migration. Utilizing advanced minimally invasive techniques ensures excellent clinical outcomes, structural durability, and an accelerated return to normal daily activities.
